PATIENT REGISTRATION

On or before your initial visit with us, we’ll need to gather some information about you. We encourage you to use our secure online portal to register. However, if you prefer to download a form to complete and bring, you may download the Patient Registration Form here.  Registering prior to your initial appointment will help to reduce the amount of time spent in our waiting room.


When you come in for your first appointment, please bring the following:

  • Photo ID
  • Insurance Card (if applicable)
  • List of any medications you may be taking, including over the counter medications or supplements.

APPOINTMENTS

We see patients by appointment. If you are unable to keep your scheduled appointment, we ask that you give us 24 hours notice so that we may open this time to another patient in need.

PRESCRIPTION REFILLS

Please allow 24 hours for us to refill any medications for you. When calling in for a prescription refill request, please have the following information available:

  • Pharmacy Name
  • Pharmacy Phone Number
  • Name of Prescription and Dosage


Please understand that we do not call in new medications or refills after hours or on weekends.

FINANCIAL

Please be prepared to pay for any co-payments, deductibles or non-covered services. For your convenience, we accept checks, cash, debit and credit cards.

INSURANCE

We accept most major insurance plans. We are actively in the process of becoming credentialed with some major health insurance plans that we have been working with for many months. Please let us and your insurance company know if we are not currently credentialed with your insurance, and we will try to become credentialed with them. We would like to provide care for you regardless of your specific insurance plan, and if alerted, we will start working on the process of becoming an in-network provider. This process can be quite frustrating for both physicians and patients, so we appreciate your understanding. If you do not see your insurance company listed below, please call the office as this list is always being updated and many plans fall under a larger umbrella plan of a different name.
